Telehealth Consent

Last updated: April 5, 2026

WE ARE NOT A REPLACEMENT FOR EMERGENCY MEDICAL SERVICES. IF YOU HAVE A MEDICAL EMERGENCY, SEEK EMERGENCY MEDICAL CARE IMMEDIATELY IN-PERSON OR DIAL 911.

What Is Telehealth?

Telehealth involves the delivery of health-related services and information via electronic and telecommunications technologies. This allows for patient–clinician interaction, diagnosis, consultation, treatment, and care management at a distance. Telehealth encounters may occur through live video, asynchronous messaging, or other digital communication methods.

Consent to Telehealth Services

By using OralTrim's platform and services, you voluntarily consent to receive healthcare services via telehealth. You understand and agree to the following:

  • Telehealth consultations are conducted by licensed healthcare providers through our affiliated provider network and are subject to the same standards of care as in-person visits.
  • The interactive electronic systems used will incorporate reasonable security protocols to protect the confidentiality of patient information.
  • Telehealth may involve electronic communication of your personal medical information to healthcare providers who may be located in other areas, including out of state.
  • You may expect the anticipated benefits from the use of telehealth in your care, but no results can be guaranteed or assured.

Benefits of Telehealth

  • Improved access to medical care by enabling you to receive services remotely.
  • Convenience of receiving care from your home without travel, time off work, or childcare arrangements.
  • More efficient healthcare evaluation and management.
  • Access to specialists who may not be available locally.

Risks of Telehealth

  • Information transmitted may not be sufficient to allow for appropriate medical decision making.
  • Delays in evaluation and treatment could occur due to technology deficiencies or failures.
  • In rare instances, security protocols could fail, causing a breach of privacy.
  • Lack of access to complete medical records may result in adverse drug interactions or other clinical errors.
  • Telehealth services may not be available in all locations or for all medical conditions.

Your Rights

  • You have the right to withhold or withdraw your consent to telehealth services at any time, without affecting your right to future care or treatment.
  • You have the right to inspect all information obtained during a telehealth interaction and may receive copies for a reasonable fee.
  • All existing confidentiality protections under federal and state law apply to information disclosed during a telehealth encounter.
  • You have the right to receive information about the provider conducting the telehealth encounter, including their qualifications and credentials.

Acknowledgment

Your continued use of OralTrim's services constitutes your understanding and acceptance of the above terms. You hereby authorize the use of telehealth in the course of your diagnosis and treatment.
