In accordance with applicable state consumer protection statutes, including Florida Statute 501.0575, the following Bill of Rights is provided to all users of OralTrim's weight management program.

WARNING: RAPID WEIGHT LOSS MAY CAUSE SERIOUS HEALTH PROBLEMS. RAPID WEIGHT LOSS IS WEIGHT LOSS OF MORE THAN 1½ TO 2 POUNDS PER WEEK OR WEIGHT LOSS OF MORE THAN 1 PERCENT OF BODY WEIGHT PER WEEK AFTER THE SECOND WEEK OF PARTICIPATION IN A WEIGHT-LOSS PROGRAM.

CONSULT YOUR PERSONAL PHYSICIAN BEFORE STARTING ANY WEIGHT-LOSS PROGRAM.

ONLY PERMANENT LIFESTYLE CHANGES, SUCH AS MAKING HEALTHFUL FOOD CHOICES AND INCREASING PHYSICAL ACTIVITY, PROMOTE LONG-TERM WEIGHT LOSS.

QUALIFICATIONS OF THIS PROVIDER ARE AVAILABLE UPON REQUEST.

Your Rights

As a participant in our weight management program, you have the right to:

1. Ask questions about the potential health risks of this program and its nutritional content, psychological support, and educational components.
2. Receive an itemized statement of the actual or estimated price of the weight-loss program, including extra products, services, supplements, examinations, and laboratory tests.
3. Know the actual or estimated duration of the program.
4. Know the name, address, and qualifications of any dietitian or nutritionist who has reviewed and approved the weight management program.
5. Receive full disclosure of any potential side effects of prescribed medications or recommended treatments.
6. Cancel your participation in the program at any time. See our Cancellation and Refund Policy for details.
7. Access your medical records maintained by your healthcare provider in accordance with applicable state and federal law.
8. Receive care from licensed providers who will make prescribing decisions independently based on your medical history and clinical evaluation.

Program Disclosure

OralTrim provides a technology platform that connects patients with licensed healthcare providers. Medical services are provided through our affiliated provider network. All prescribing decisions are made independently by licensed clinicians based on your medical history, current health conditions, and clinical evaluation.

Results vary based on starting weight, program adherence, diet, and exercise. Treatment is subject to provider approval. Medication is not guaranteed. Compounded medications, if prescribed, are produced in FDA-regulated facilities but are not FDA-approved or evaluated for safety, efficacy, or quality.
